Dinner Reservation Form: 

For phone call bookings only. Guests who have reserved through our online system do not need to complete this form.

There are 2 choices for the main course. 1 Fish & 1 Meat. Please indicate if there are any choices a particular guest will not eat from this list: Pork Chop, Duck, Chicken, Lamb, Salmon, Cod. ex guest A) No duck or tomatoes, guest B) No duck or salmon)

Please note that expressing a dislike does not mean it will not be one of the two main course options offered for dinner on the night of your reservation. Our aim is to have at least one of the two dishes being offered that evening be a dish a guest is content to order. Because we are aiming to accommodate up to 50 guests in a night, we ask that you only mention severe dislikes. If your party lists multiple fish and meats from our list as dislikes, we may not be able to accommodate you the night of your booking. If we foresee an issue once the menu is set, we will do our best to contact you in advance to either change your date to a night with a menu that works or give you the opportunity to cancel your reservation before the 24 hour cancellation period. Menus are determined a night or two prior based on repeat guests, allergies and food supplies. 

Ex in a party of 4: 2 Guests NA (N allergies) 1-celiac 1-gluten sensitive (but can have some) & lactose intolerant (but butter is ok).


Should you list the following restrictions: Lactose/dairy, gluten, vegan, garlic, onion, egg, nuts, corn. Please review the bottom of this form to ensure you have provided necessary details around those restrictions: 

 

Tables are first come first serve, but we can ensure a seat on the main floor if there is a mobility issue. The washroom is located off the main level. There are about 14 steps (with railing on left going up) to get to the second level.

Are there any details you'd like us to be aware of: Special needs, Anniversaries, Birthdays...etc.

Dining With Minors:  Guests under 19 may not dine at the 8:00pm sitting due to our liquor license rules. Please note that the Twine Loft is not a family style-restaurant, however, we can accommodate younger guests willing to eat from our regular set price menu who are able to 

  1. Remain seated for 1.5-2 hours (regretfully, leaving to play outside between courses is not possible)
  2. Can express themselves at a volume appropriate for an intimate fine dining setting.

WE OFTEN REQUIRE FURTHER DETAIL WHEN ANY OF THE FOLLOWING ALLERGIES AND SENSITIVTIES ARE LISTED. PLEASE EDIT THE ALLERGIES SECTION IF ANY APPLY TO YOU

Celiac: If you indicate celiac no gluten will be used in the preparation of your meal. Please note that we cannot guarantee zero cross contamination, but we will be very mindful.

Gluten Intolerant: To us this means gluten bothers you, you don't wish to have it served to you, but it is not an allergy. If you are ok with some gluten (like a light flouring of meat or you are ok with it in a dessert, but you don't want have a 3 course meal with lots of gluten) please let us know.

Lactose Intolerant: If you lists someone as lactose intolerant please let us know if that includes butter as this is often used in our dishes. Better understanding the types of food you can eat (no cheese or some cheese, no cream, some cream/milk but not a heavy creamy sauce..) is helpful to us.

Seafood: if you indicate seafood please specify if this is both fish and shellfish and the extremity of the allergy. Can you be in an open kitchen restaurant if it is being cooked for others. We do not typically cook shellfish in our restaurant.

Vegan: If someone identifies as vegan we interpret this to mean that guest will not eat any meat, dairy (eggs, butter, milk/cream) or bi-products of animals (ex chicken broth). As a set menu restaurant creating a 3-course vegan menu is more challenging than a vegetarian menu.  Please only identify as vegan if you meet the description above. We are happy to accommodate but please dop not indicate vegan unless you strictly follow the diet outlined above.  If you are often a vegan but do make exceptions consider listing yourself as a vegetarian or pescatarian. 

Vegetarian: If someone identifies as a vegetarian we will offer a menu that does not include meat OR fish, but it may contain products from animals such as milk, eggs.

Pescatarian: If someone in the reservation does not eat meat, but will eat fish please identify them as a pescatarian on our forms.

Onion, Garlic: These are two essential ingredients to cooking and ones we often cannot remove from our soups and main dishes.  If you list either ingredient please be very specific about what you mean (ex dislikes raw onion and heavy garlic). Should you list them under allergies or sensitivities, please specify which it is and Indicate if onion powder or garlic power are ok.

Egg: If you list egg under allergy / sensitivity please indicate which it qualifies as and if you can have it baked into items or not (ex cake) Egg may be brushed over certain proteins before baking, in a house made mayo or appear in baked goods. 

Nuts: Should you list nuts as an allergy please indicate severity and which specific nuts are an issue.  Please not there are nuts on our premises.  If someone has a severe nut allergy we will aim to avoid having nuts on the menu that evening to reduce the risk for the guest.  

Corn: We don't typically serve corn, but corn starch is an ingredient we use.  If cornstarch is an issue please indicate.
